REQUESTING MEDICATION
Patients are now able to use the NHS App to order repeat prescriptions, or to email their prescription requests directly to our team. When emailing requests, please ensure that you include your name, D.O.B and the medication that you are requesting. Requests for medication will be processed within five working days.
CAR PARKING REMINDER
Our car park is independently monitored and a permit will need to be issued for the duration of your appointment. This can be done by inputting your registration details into the tablet in reception.

- The date that the extraction of data will begin is the 31st March 2022. Should you wish to opt out of this program, please contact the practice by phone stating your wish to do so, or download and complete this 'opt-out' form (docx file) and leave it with reception.
Covid Vaccine Status
Proof of your vaccination status can now be found on the NHS App. This can be downloaded for IOS and Android through the App / Play Store. Alternatively you can use the following links to download the IOS app, or download the Android app.
If you cannot access the NHS App, you can call the NHS helpline on 119 and ask for a letter to be posted to you. This must be at least 5 days after you have completed the course of the vaccine. The letter may take up to 5 days to reach you.
Please note: we are unable to issue this certificate at the practice.
